Chronic Fatigue Syndrome / Myalgic Encephalomyelitis (CFS/ME)

Also known as: Chronic Fatigue Syndrome, Myalgic Encephalomyelitis, ME/CFS, Systemic Exertion Intolerance Disease, SEID

Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) is a complex, chronic, multisystem disorder characterised by debilitating fatigue, post-exertional malaise (PEM), unrefreshing sleep, cognitive dysfunction and orthostatic intolerance. Diagnosis is clinical after appropriate exclusion of alternative medical and psychiatric conditions, and there is currently no curative treatment. Evidence-based management focuses on pacing, symptom management, optimisation of sleep, treatment of comorbid conditions, nutritional support where indicated and multidisciplinary care. In Chinese medicine, presentations commonly correspond with Xū Láo (虚劳) patterns involving Spleen Qi deficiency, Heart-Spleen deficiency, Qi and Yin deficiency, Liver Qi stagnation with Blood deficiency and Kidney Essence deficiency. TCM care is supportive and adjunctive only, must remain below the patient's post-exertional threshold, and must not replace medical investigation or ongoing physician management.

    Lifestyle & Diet

    • •Use pacing and stay within the energy envelope rather than pushing through with graded overexertion.
    • •Avoid activities that consistently provoke post-exertional malaise, and watch for delayed symptom worsening after physical or cognitive effort.
    • •Prioritise restorative sleep with a consistent routine, and treat comorbid sleep disorders.
    • •Maintain adequate hydration and salt intake where orthostatic intolerance is medically confirmed.
    • •Optimise nutrition with regular, easily digested meals.
    • •Address comorbid pain, anxiety or depression with the treating doctor.
    • •Individualise activity: no fixed incremental exercise programmes; keep an activity and symptom diary.

    Cautions & Contraindications

    See sources
    • •Avoid aggressive acupuncture technique — strong stimulation may trigger prolonged post-exertional malaise. Use gentle treatment only.[]
    • •Do not claim that acupuncture or Chinese herbal medicine cures ME/CFS; use supportive care language only.[]
    • •Do not recommend exercise beyond individual tolerance or fixed graded exercise programmes; follow pacing principles.[]
    • •Avoid restricted herbs including Má Huáng (Ephedra) and Fù Zǐ (Aconite), and review all herbal prescribing against current medications.
    • •Observe standard pregnancy acupuncture precautions.

When to reassess or seek care again

    See sources
    Review timelines
    • •Initial visit: confirm diagnostic criteria, document post-exertional malaise severity, assess sleep quality, evaluate orthostatic symptoms and review medications and comorbidities.
    • •Ongoing: monitor functional capacity, review pacing adherence and assess symptom fluctuations.
    • •Re-screen for alternative diagnoses if the presentation changes or new red flags appear.

Useful outcome measures include the Fatigue Severity Scale, SF-36 physical function, a post-exertional malaise questionnaire, a patient activity diary and quality-of-life measures.
