If something on Medi-Chi looks wrong, unsafe or out of date, tell us here. Every report is read, triaged and logged, and anything that changes the clinical content shows up on the public AI changes & verification log.

This form is not for medical emergencies. If you or a patient needs urgent care, call your local emergency number (000 in Australia, 911 in the US, 999 in the UK) or go to the nearest emergency department. Do not include patient-identifying details in a report.
