    Piriformis Syndrome (Deep Gluteal Pain)

Also known as: deep gluteal syndrome, piriformis-related sciatica, buttock pain with leg radiation

Piriformis syndrome is deep buttock pain, sometimes radiating down the back of the leg, caused by irritation of the sciatic nerve near the piriformis muscle. In Chinese medicine it follows the Gallbladder and Bladder channels and is treated as channel obstruction from overuse, prolonged sitting or cold. Acupuncture is used as an adjunct to stretching and activity modification.

    Cautions & Contraindications

    See sources
    • •Leg pain with progressive weakness or bladder changes must be medically assessed first — lumbar disc causes must be excluded.[]
    • •Deep needling at GB30 requires anatomical care near the sciatic nerve — use trained technique.[]
    • •Chinese medicine is adjunctive and does not replace neurological assessment where red flags exist.[]

    Frequently Asked Questions

Is piriformis syndrome the same as sciatica?+

No. Sciatica most often comes from a lumbar disc, while piriformis syndrome is irritation of the sciatic nerve in the buttock. They can feel similar, so proper assessment matters before treatment.

    Can acupuncture help piriformis syndrome?+

Acupuncture along the Gallbladder and Bladder channels is a common adjunct for deep gluteal pain, alongside stretching and activity modification.
