Rotator Cuff Impingement (Subacromial Pain Syndrome)

Also known as: subacromial pain syndrome, shoulder impingement, rotator cuff tendinopathy, painful arc syndrome

Rotator cuff impingement causes shoulder pain, especially when lifting the arm overhead or lying on that side. In Chinese medicine it is treated as obstruction of the channels crossing the shoulder, often from overuse, injury or cold exposure. Acupuncture is used as an adjunct to exercise-based rehabilitation, which has the strongest evidence.

    Lifestyle & Diet

    • •Follow a graded rotator cuff strengthening program — exercise has the strongest evidence for this condition.
    • •Avoid repetitive overhead work during the painful phase; modify rather than stop activity.
    • •Apply warmth before exercise and avoid sleeping on the painful shoulder.

    Cautions & Contraindications

    See sources
    • •Suspected full-thickness tear (sudden weakness after injury) needs imaging before manual treatment.[]
    • •Chinese medicine is adjunctive — it does not replace physiotherapy or orthopaedic assessment.[]
    • •Avoid aggressive needling into the subacromial space; use standard safe depths.[]

    Frequently Asked Questions

When should a sore shoulder be scanned?+

Refer for imaging if there is sudden weakness after an injury, an inability to lift the arm, or pain that fails to improve with a structured rehabilitation program.
